Individualizing Treatment Decisions in MG

Effective MG care requires treatment goals that reflect not only disease activity but also each patient's priorities, responsibilities, and expectations.

In ‘Individualizing Treatment Decisions in MG,’ our panel explores how individual treatment goals and patient preferences shape therapeutic decision-making in myasthenia gravis. Expert faculty emphasize that treatment priorities vary considerably according to age, employment, lifestyle, personal values, functional expectations, and tolerance for potential adverse effects.

The panel illustrates how patients with similar disease severity may have very different treatment goals. Some may accept mild residual symptoms in exchange for a lower medication burden, whereas others may require near-complete symptom control because of occupational responsibilities, travel, or other daily demands. The experts stress the importance of discussing both efficacy and adverse effects so that treatment decisions reflect a balanced understanding of potential benefits and burdens.

Reproductive planning is also highlighted as an important consideration for women of childbearing potential, particularly when treatment choices may affect pregnancy planning. Financial burden, travel to infusion centers, treatment frequency, caregiver responsibilities, and the impact of therapy on daily life are similarly important components of shared decision-making.

The panel also discusses the psychological and social consequences of living with a chronic disease, including anxiety, depression, employment concerns, and financial stress. Importantly, treatment goals may change as disease control improves. Patients initially focused primarily on symptom relief may later prioritize convenience, independence, and reduced treatment burden. The experts therefore emphasize periodically reassessing goals throughout the disease course.

Our next episode, ‘Understanding MG Pathophysiology and Therapeutic Targets,’ examines the immunologic pathways underlying MG and how increasing mechanistic understanding has enabled more precise therapeutic targeting.
