Understanding Myasthenia Gravis Pathophysiology and Therapeutic Targets

Decades of MG biology are now informing increasingly precise strategies that intervene at different points along the autoimmune pathway. Supported by Vor Bio Medical Affairs.

This episode, titled ‘Understanding MG Pathophysiology and Therapeutic Targets,’ features panelists examining how advances in disease biology have informed the development of more selective treatment strategies in myasthenia gravis. The experts review longstanding knowledge of thymic involvement, T-cell activation, B-cell stimulation, pathogenic autoantibody production, complement activation, and neuromuscular junction injury.

The panel explains that clinicians have historically influenced these pathways through broad immunosuppressive approaches, corticosteroids, nonsteroidal immunosuppressants, intravenous immunoglobulin, and plasma exchange. More recently, advances in therapeutic development have enabled clinicians to intervene at increasingly specific points within the disease pathway.

The faculty distinguish downstream approaches, which reduce or interfere with circulating antibodies and their effector mechanisms, from upstream strategies intended to decrease antibody production at its source. They emphasize that upstream targeting does not represent a single mechanism. Potential strategies include targeting specific B-cell populations, interfering with factors that support B-cell growth and survival, or eventually intervening even earlier in the loss of immune tolerance.

The discussion also highlights an important challenge associated with greater immune specificity: clinicians must suppress pathogenic immunity while preserving protective immune functions, including responses to infection and vaccination. Panelists note that therapeutic advances may now be outpacing aspects of basic understanding of neuromuscular junction immunology, reinforcing the continued importance of translational research.

In the next episode, ‘Managing Corticosteroids and Immunosuppression in MG,’ panelists will examine established treatment approaches, corticosteroid use and tapering, steroid-sparing therapies, and the balance between efficacy and treatment-related toxicity.
