One of the earliest concepts we learn in physiology is homeostasis, the body’s capacity to maintain internal stability despite constant external change. In complex organisms, that balance is dynamic, individualized, and influenced by both biological and environmental factors. Maintaining homeostasis is not a static achievement, but a continuous process shaped by a person’s physiology and surroundings.
In today’s health discourse, the notion of “wellness” is often an oversimplified way to maintain a healthy homeostasis. Doubtless, wellness is essential, yet often it doesn’t capture the individual complexity of maintaining homeostasis, especially for patients living with a chronic condition.
Migraine is a particularly illustrative example of this balancing act. Managing migraine requires careful attention to a patient’s individual make-up and needs, often beyond what lifestyle adjustments alone can achieve.
Lifestyle Helps, But It’s Not the Whole Story
Healthy habits are crucial. Adequate sleep, regular hydration, balanced nutrition, routine physical activity, and stress management all contribute to overall health and can reduce the burden of migraine. They form the foundation of good migraine care and remain an essential part of long-term management.
However, migraine is a complex neurological disease. Healthy habits alone may not always be enough to keep it under control.
This is reflected in clinical practice. The American Headache Society recommends considering preventive treatment for patients whose migraine attacks are frequent, disabling, or significantly interfere with daily life.1 Lifestyle modifications remain essential, but prevention becomes necessary when the disease continues to affect quality of life despite those efforts.
Research also illustrates the complexity patients often face. Although regular exercise supports overall health and may help reduce migraine burden for many, 38% of individuals with migraine report exercise-triggered attacks, and more than half ultimately stopped the activity altogether.2
The same complexity applies to other healthy behaviors. A balanced diet supports overall health, but foods that are well tolerated by most people may still act as individualized triggers for others.
Importantly, the message is not that lifestyle changes are ineffective. Quite the opposite. They are a cornerstone of migraine management. But for many people living with migraine, especially those whose attacks remain frequent and/or disabling, lifestyle alone cannot fully address the underlying neurobiology of the disease. Those patients deserve additional options that complement healthy habits rather than replace them.
The Unpredictable Nature of Migraine
Migraine management is further complicated by the sheer unpredictability of prodromal symptoms and attacks. Even when patients believe they understand their patterns, studies show that most are poorly able to predict the timing of the next attack, and simple models accounting for short history of patterns often do not match objective data.3 An AI model accounting for individual history over a whole month is much more accurate in assessing the likelihood of the next migraine attack.4 Our brains are not built for such long-term data comprehension, and therefore should rely on digital tools to assess the likelihood of an upcoming migraine attack.
Beyond lifestyle, environmental and biological factors, like barometric pressure or hormonal changes, are beyond anyone’s control. Even lunar cycles may play a role.

As a system, healthcare often excels at acute care but struggles with chronic conditions. The reasons are structural and may include:

A Self-Regulatory Approach
The goal of migraine therapy is straightforward in principle, but complex in execution: reduce the frequency, severity, duration, and impact of attacks while minimizing side effects and preserving quality of life.
Historically, most treatments have followed a pharmacologic approach: Chemically target receptors or alter neurotransmitter activity to interrupt pain signaling pathways.
While effective, these targets often present throughout multiple systems in the body, which is why systemic side effects such as GI irritation, kidney strain, sedation, cognitive effects, etc., are common even with everyday medications such as NSAIDs and prescribed triptans.
The nervous system, however, communicates through two fundamental "languages": chemistry and electricity. Most migraine therapies have focused on the chemical side of that equation via pharmacological drugs.
Advances in neuromodulation, such as remote electrical neuromodulation (REN), offer a different pathway, allowing us to engage the nervous system through its electrical signaling without chemicals.
REN uses targeted peripheral nerve stimulation to activate a well-characterized descending pain-modulating pathway.5 By electrically stimulating A-delta and C nociceptor fibers in the upper arm, REN:
- Activates brainstem regions involved in pain inhibition
- Promotes release of endogenous neurotransmitters such as serotonin and norepinephrine
- Engages conditioned pain modulation
- Reduces central sensitization
Evidence, from randomized controlled trials, open label studies, and real-world data, published in more than 35 original peer-reviewed publications, including data from more than 58,000 patients, consistently shows that REN can achieve effects similar to both acute and preventive pharmacologic treatments, but without introducing external chemical compounds that circulate systemically.6,7 Instead, it harnesses intrinsic neural circuitry to help restore homeostatic balance and regulate abnormal pain processing.
This is not an alternative to medicine. Rather, it is medicine delivered by engaging the body's own electrical signaling and pain-regulatory mechanisms.
Integrating Wellness with Innovation
Lifestyle and medical intervention are often discussed separately, but in practice, they are most effective when used together. Lifestyle optimization lays the groundwork, and regulated treatments of drugs and medical devices provide the additional support patients need when wellness and lifestyle alone cannot shield them from migraine. Together, they create a symbiotic approach that acknowledges both the value of healthy habits and the necessity of science-driven innovation.
A Healthier Definition of Wellness
Wellness is not a symptom checklist or absence of disease. For people living with migraine, wellness means having the right strategies and tools to help the brain return to homeostatic function as quickly and safely as possible.
Needing a treatment intervention does not diminish a person’s wellness. It acknowledges the reality that human physiology varies, and some individuals require more than lifestyle adjustments to maintain balance.
Evidence-based innovation is at the core of progress in our society, and the same principles should apply to health.
Homeostasis is the goal. The future of care is finding better ways to help the body get there.
