This episode focuses on vamoralone, the newest corticosteroid option for DMD. Dr. Jayaraman provides a detailed overview of the drug, describing it as a dissociative corticosteroid with possible mineralocorticoid antagonist activity. It was developed to address persistent concerns about growth suppression and bone health that remain even with deflazacort.
Two pivotal studies are reviewed: a 2022 trial examining 24-week outcomes and a 2024 crossover study extending follow-up to 48 weeks. Two doses were compared—2 mg/kg and 6 mg/kg—with the higher dose demonstrating superior motor outcomes, particularly over longer follow-up. The primary endpoint, time-to-rise velocity from the floor, was met. Key secondary motor endpoints were also achieved. Critically, vamoralone showed comparable efficacy to prednisolone (the active control) but with a significantly improved safety profile, particularly regarding bone biomarkers and linear growth.
Dr. Brendsema acknowledges that while improved height is socially and emotionally meaningful for young school-age children, the field holds a nuanced view on growth: being physically smaller may actually confer mechanical advantages in terms of ambulation and caregiver support for non-ambulatory patients. He also raises the point that BMI may not be the ideal outcome measure, as patients appear to be growing both taller and heavier.
Dr. Veerapandiyan addresses vamoralone's mineralocorticoid receptor antagonist properties, noting theoretical cardiac benefits similar to those of spironolactone or eplerenone. However, clinical trials excluded patients on those agents, so the interaction remains unstudied. In practice, vamoralone is used alongside—not as a replacement for—existing cardiac medications, with close electrolyte monitoring. The panel agrees that further data are needed to clarify its cardiovascular impact.
